The LIB Therapeutics direct-to-patient program (the "Program") is available to patients at least 18 years old who have been prescribed LEROCHOL by their physician and who pay out-of-pocket with cash or credit card for LEROCHOL. By participating in this Program, you agree to these terms and conditions.
You will not use insurance to pay for LEROCHOL by submitting any claim for reimbursement to insurance. This includes Medicare Part D, a Medicare Advantage prescription drug plan, Medicaid, and any other federal or commercial insurance policy. You will not count any out-of-pocket costs toward your insurance deductible or true out-of-pocket costs under a Medicare Part D Plan. You will purchase all of your LEROCHOL prescriptions during the year using this Program. The cash price for LEROCHOL is $199 for a one-month supply. It is possible that the amount that you pay for LEROCHOL under this Program could be more than what you would pay if you used insurance.
LIB Therapeutics has partnered with HealthDyne, LLC, a licensed pharmacy, to fill and ship your prescription for LEROCHOL to you. Your physician will need to submit a prescription for LEROCHOL to HealthDyne. Your purchase of LEROCHOL will be subject to HealthDyne's terms and conditions of fulfillment. You should contact HealthDyne about any issues with your product fulfillment.
LEROCHOL is only available to residents of the United States. You are not under any obligation to continue purchasing LEROCHOL. If you have any questions about LEROCHOL, you should contact your prescribing physician. LIB Therapeutics reserves the right to modify or discontinue the Program for any reason at any time without prior notice. These terms are governed by the laws of Ohio. Any dispute arising out of or relating to these terms, the Program, or your purchase of LEROCHOL through the Program, will be governed by the dispute resolution provisions in Section 16 of our Website Terms of Use.
LEROCHOL is a prescription injectable medicine used along with diet and exercise to reduce low-density lipoprotein cholesterol (LDL-C), or “bad cholesterol,” in adults with hypercholesterolemia (high cholesterol), including heterozygous familial hypercholesterolemia (HeFH). It is not known if LEROCHOL is safe and effective in children.
Before starting LEROCHOL, tell your healthcare provider, including your pharmacist, about all your medical conditions, including if you are pregnant or plan to become pregnant, are breastfeeding or plan to breastfeed, or if you take other medicines, including prescription and over-the-counter medicines, vitamins, and herbal supplements.
Use LEROCHOL exactly as your healthcare provider tells you to use it. LEROCHOL is a subcutaneous injection (under the skin) self-administered one time every month. If your healthcare provider decides that you or a caregiver can give the injections, you should receive training on the proper injection technique. Do not stop using LEROCHOL without talking to your healthcare provider, as cholesterol levels may increase if treatment is stopped.
The most common side effects of LEROCHOL include injection site reactions such as redness, itching, swelling, pain, bruising, or tenderness, runny or stuffy nose and sore throat (nasopharyngitis), diarrhea, nausea, and swelling of the arms, hands, legs, or feet (peripheral edema). These are not all the possible side effects of LEROCHOL. Tell your healthcare provider if you have any side effect that bothers you or does not go away.
Call your doctor for medical advice about side effects.
